Published Reviews

The learning and good practice that emerges from Serious Case Reviews and other investigations is shared at a national and local level to maximise the opportunities for learning.

On 6 February 2023 the TSSP in conjunction with Manchester Safeguarding Partnership published the LCSPR in relation to N, B and YK.

This thematic local child safeguarding practice review was jointly commissioned by the two safeguarding partnerships to ensure the tragic cases concerned were rigorously examined and any lessons were captured and acted upon.

Our heartfelt sympathies go out to the families and friends of these children. We know that it will be scant consolation to them to know that the task of doing all we can to bolster safeguarding practice is one which is treated with the utmost seriousness by all involved.

While the review did not identify any specific shortcomings which suggested that the tragic events which befell the children were preventable, the learnings identified for the Partnerships are being implemented in full to promote best practice with the children and families whose lives are affected by youth violence and exploitation.

On 8 June 2022 the TSSP published a LCSPR in relation to Child Michael.

This review relates to the tragic death of Michael, a 17 year old young man who sadly died of ingesting a large quantity of medication that had been prescribed to his parents. Trafford Strategic Safeguarding Partnership offer sincere condolences to Michael’s family and to those who knew him.

This important review makes a number of recommendations to agencies who provide services to children and adults in Trafford and for which an action plan is being developed. The TSSP will review the action plan to ensure that progress is being made to strengthen safeguarding practice.

On 16 May 2022 published their Child Safeguarding Practice Review in relation to children Teddy, Wilbur and Peter. This thematic review has provided important learning for agencies who provide services to children and young people in Trafford. The review relates to three young people. One of the young people tragically died and our sincere condolences go to their family and friends. 

TSSP welcomes the recommendations made by the Independent Review and an action plan is in place to address these. The TSSP will review the action plan over the coming months to ensure that progress is being made to strengthen safeguarding practice.

Serious Case Review - Baby X

On 16 December 2019 the TSSP published its Serious Case Review in respect of Baby X.

Our sincere condolences go to the family of Baby X. The Strategic Safeguarding Partnership welcomes the recommendations made by the Independent Review in this tragic case, and an action plan is in place to address them. We will review actions in the next 3 months to ensure that progress is being made to strengthen the safeguarding of children such as Baby X.
